Our Home Hospital (HH) service enables eligible patients to be transferred home and receive hospital standard care to complete their recovery. The service is managed by the SMHS Community Services program and was formerly known as Hospital in the Home (HITH).

Home Hospital is a hospital substitution service replacing a full hospital admission or a component of a hospital admission for suitable patients. HH provides acute inpatient equivalent care, utilising highly skilled staff, hospital technologies, equipment, medication, and safety and quality standards, to deliver hospital-level care within a person’s place of residence or preferred (non-hospital) treatment location.

SMHS delivers quality, safe and effective hospital and health services within a catchment area stretching more than 3300 square kilometres across the southern half of Perth.

The Consultant Physician position provides clinical leadership and will provide support to develop and advocate for the HH service. This position will lead clinical decision making for patients on the HH caseload. They will have input into expanding patient pathways, management plans, risk assessments and escalation if required. Due to the nature of the HH position dual training (e.g., Medical/Geriatrics/Infectious Diseases/ Cardiology/Endocrine/Renal) is highly regarded. Physicians from any of these specialties with generalist interest are encouraged to apply.

The Consultant role is based on site at the hospital, ‘referral finding’ i.e. working ED and AMU to identify suitable patients, accepting patients, providing service advocacy and relationship building with other inpatient teams. Review of inpatients in the community will generally be done virtually.
