Medical Lead – Paediatric – Transition Service

This is a development opportunity being offered as a Senior Medical Practitioner Year 1-9. Level and Increment will be determined by the successful applicant’s skills, knowledge, and ability in the role.

 

This is a Fixed Term sessional appointment at 4 Sessions / 16 hours per fortnight (0.2 FTE) for 5 years with the possibility of extension(s).

The main focus of the Medical Lead of the Child and Adolescent Health Service (CAHS) Transition Service is to provide clinical leadership and strategic oversight for the safe, coordinated, and developmentally appropriate transition of adolescents and young people with chronic and complex health conditions from paediatric to adult health services. 

The Medical Lead works collaboratively with external stakeholders including consumers to establish best practice and develop resources to assist CAHS patients in the process of transition.

In this role, you will:

• Ensure the CAHS Transition Service is high quality, best practice and outcome focussed in line with CAHS, State and National policies. 

• Promote shared decision-making and health literacy for CAHS adolescents and young adults

• Support integration between paediatric and adult services, including shared-care and co-management mode

• Continue to develop the CAHS Transition Service’s consult liaison service to support transition planning for adolescents and young people with chronic and complex health conditions
